There are a lot of things here, so let me do them one at a time.

I may have made a lot of people who have nuts feel better just by that one sentence.

Let me also say this to help normalize things for people who have nuts, one usually hangs lower than the other. And usually it’s the left. Though if it’s the right, that’s totally normal too.

So just to sum up, nuts are often different sizes and usually don’t hang at the same level.

Now let’s go back to the size difference from your question. Is it a small egg? A big egg? A small grape? Usually there’s a difference, about half a teaspoon difference. Like two grapes but one slightly larger than the other.

What you’re describing may be a sign of something. It could be an infection, or a hydrocele, or a varicocele.

If it’s an infection, then there’s usually swelling.

If it’s an infection related to an STI then there’s usually pain when you pee. And a discharge from your penis.

A hydrocele is when there’s a buildup of fluid around a testicle. If it’s this, then your scrotum will feel heavy. And there’s often pain with a hydrocele.

A varicocele is when the veins in the scrotum get larger. This can also feel like a heaviness in your scrotum. And one of the things that’s common with a varicocele is the scrotum feeling like a bag of worms. Or spaghetti.

I have to mention the possibility of testicular cancer, if for no other reason than I think that we don’t talk about it enough. I’m not saying that’s what this is! But you mention lack of pain. And lack of pain does not rule out things like testicular cancer.

The size of one testicle changing is often a sign of testicular cancer. If it’s cancer, you may or may not feel a lump in your nuts. And there may be a feeling of heaviness in the scrotum. You may also feel a dull ache in the lower back or lower stomach area.

But you may not get all of these. But the size changes in the nuts is something to be concerned about if it’s new.

So the one thing you need to ask yourself is, is this size difference new? If it is, then I strongly suggest you talk to a health care provider.

If it’s nothing, then you’ll feel better hearing that. If it is something, it’s best to get it taken care of sooner rather than later.
